Coaches Assessment Sheet
Varsity Coach:
I would like to personally thank you for allowing us the privilege
of serving you this season. As the secretary for our chapter I would like to
get your assessment on our performance this season. I am asking that you take
a few moments and complete the attached sheet and send it back to me, the day
after the game. Win or Lose try to be as honest as possible. In order to protect
your privacy, I will not share your assessment sheet with the officials involved
in this game. The results will be used to try to improve the overall performance
of our officials.

- _________Officials controlled game.
The last three items deal with the communication skills of the official. Does
the official effectively communicate with the individuals involved in the game?
Is the official approachable? Does the official, in a timely manner, define
what the problem is, determine the proper action, and resume play?
